Vaughn, William S.* (5/16/06) The physician’s DC medical license was revoked for prescription irregularities.
Inocencio, Narciso* (5/8/06) The physician’s DC medical license was revoked as a result of disciplinary action in Maryland for prescription irregularities.
Kioko, Gideon M.* (7/10/06) The physician’s DC medical license was revoked as a result of disciplinary action in Maryland for quality of care issues.
Faisal, Imran* (7/10/06) The physician’s DC medical license was revoked as a result of disciplinary action in New York related to a guilty plea for petit larceny and filing false statements with several health facilities in New York.

Summarily Suspended
Bassett, David W.* (4/26/06) The physician’s DC medical license was summarily suspended for conduct that represented an imminent danger to the health and safety of the residents of the District of Columbia. The physician’s Maryland license was revoked as a result of prescription irregularities, including prescribing drugs in quantities generally considered to be the lethal limit.
Cohen, Joel* (8/30/06) The physician’s DC medical license was summarily suspended for conduct that represented an imminent danger to the health and safety of the residents of the District of Columbia. The physician’s Maryland license was suspended on July 14, 2006 for failing to keep adequate medical records, prescribing dangerous drugs inappropriately, engaging in repeated, egregious and longstanding violations of professional boundaries with the same patient to whom he inappropriately prescribed dangerous drugs, committed other boundary violations with other patients, and committed immoral and unprofessional conduct in the practice of medicine - Order vacated by Administrative Law Judge 10/1/06.
Hackney, David T.* (10/25/06) The physician’s DC medical license was summarily suspended for conduct that represented an imminent danger to the health and safety of the residents of the District of Columbia. The physician is addicted to or habitually abused narcotics or other controlled substances, wrote prescriptions without a current DEA registration and prescribed drugs when not authorized to do so.

Tullner, William* (5/4/06) The physician’s DC medical license was suspended indefinitely upon voluntary surrender as a result of a medical condition that adversely affects his ability to practice medicine.

Salerian, Alen* (10/25/06) The physician was placed on a minimum of two year’s probation, reprimanded, fined $3,500 and required to take a course in medical ethics as a result of disciplinary action in Maryland as a result of ethical violations.

Slatkin, Stephen E.* (5/31/06) The physician’s DC medical license was placed on three years probation. The physician was also reprimanded and required to have a chart review, a psychiatric evaluation and complete CME. The physician was disciplined in Maryland for boundary violations.
Dworsky, Lynn S.* (2/27/06) The physician’s DC medical license was placed on probation for one year nunc pro tunc to June 7, 2005 or until released from probation by the Maryland Board of Physicians. The physician was reprimanded, required to undergo peer review, and complete a medical recordkeeping course as a result of disciplinary action in Maryland related to quality of care.
Pishdad, Bahram* (2/27/06) The physician’s DC medical license was placed on three years probation nunc pro tunc to June 22, 2005 or until released from probation by the Maryland Board of Physicians. The physician was reprimanded, required to undergo peer review after twelve and twenty-four months, and required to complete a course in anticoagulation therapy as a result of disciplinary action in Maryland related to quality of care.

Jackson, Hampton* (5/31/06) The physician’s DC medical license was placed on probation nunc pro tunc to January 22, 2004 until such time as he has met all the terms and conditions of his Maryland order. The physician was disciplined in Maryland for quality of care issues.
Nwosu, Okenwa* (9/27/06) The summary suspension of the physician’s DC medical license on February 25, 2005 that was predicated on an action by the Maryland Board of Physicians was terminated. The physician was placed on probation for two years nunc pro tunc to July 7, 2006. At the end of the two years, the respondent may petition the DC Board of Medicine for termination of the suspension. The termination of the summary suspension and probation reflect similar resolution by the Maryland Board of Physicians.

Malakoff, Gary A.* (6/30/06) The physician was fined, reprimanded, prohibited from examining or treating patients for eighteen months and reporting requirements by consent order as a result of false statements on renewal applications.

Adrian, Robert* (11/29/06) The physician was reprimanded and fined for practicing with an unlicensed person.

MacKoul, Paul J.* (6/28/06) The physician was reprimanded by consent order. He was charged with filing a false statement with the Board on his renewal form.

Baidey, Allen* (11/27/06) The physician was fined as a result of similar disciplinary action by the Florida Board of Medicine for a wrong site surgery.
Sanders, Reginald* (12/20/06) The physician was fined for filing a false renewal application.

Remediation
Rogers, Allan S.* (12/14/06) The physician agreed by consent order to complete ten hours of CME related to the diagnosis and treatment of non-alcoholic fatty liver disease. The Board opined that the physician should have tested a patient’s liver on a more routine basis for possible drug effects. No adverse outcome was definitively established as a result of the physician’s practice.
