2005 Orders

Revoked

Walsh, Jessica C.* (7/14/2005) The license was revoked. In the violation of the Board’s order, licensee relapsed into alcohol abuse/use and failed to comply with the treatment program and was dismissed from the treatment.

Suspended  

Hebron, Ron E.* (11/17/2005) DC Respiratory Care license was suspended and fined $1000.00 for failure to comply with continuing education (CEU) requirements.

Rivera, Leonard M.* (10/17/2005) DC Respiratory Care license was suspended and fined $500.00 for failure to comply with continuing education (CEU) requirements.

Haskins, Lavelle* (10/17/2005) DC Respiratory Care license was suspended and fined $500.00 for failure to comply with continuing education (CEU) requirements.

Akers, Nathaniel L.* (10/17/2005) DC Respiratory Care license was suspended and fined $500.00 for failure to comply with continuing education (CEU) requirements. 

Beidleman, Calvin* (12/12/2005) DC Respiratory Care license was suspended and fined $500.00 for failure to comply with continuing education (CEU) requirements.

Thorne, Sydney E.* (11/14/2005) DC Respiratory Care license was suspended and fined $500.00 for failure to comply with continuing education (CEU) requirements.

Probation

Booker, Michael R.* (3/22/2005) DC Respiratory Care license was placed on probation for a period of two (2) years from the date of the consent order which was March 22, 2005 for addiction to habitual abuse of narcotic or controlled substance.
