How do I notify you of a change in my address?

District of Columbia Law and Regulations require that you notify us within thirty (30) days of a change in your address. Your notification must include your name, profession, license number (if licensed), social security number, date of birth, and both your old and new addresses. You may notify us in one of two ways:
 
  1. Call our automated phone attendant (available 24 hours a day, 7 days a week) at (888) 204-6193 or fax (301) 459-7359.
  2. Write to:
